The Lead supports the Manager and a team of Coordinators, Advisors and Case Managers and liaisons with agency partners, Outside Paid Resources (OPR) and Transfer Payment Agencies (TPA) to explore potential residential/non-residential adult supports and services in line with the supported person’s goals.The incumbent guides and supports the areas of Transitional Age Youth, Adult Case Management and Quality Assurance to build program awareness, supports, resources and services to increase ongoing development and capacity building of the program to enhance the overall quality of life of service users.Primary ResponsibilitiesProvide guidance and support to the Team through regular discussion, coaching, mentoring, work planning to support goal attainment and achieve work plan outcomes.Identify and evaluation risks associated with program activities and collaborate with the Team and Manager to take appropriate action.Lead the development of individualized adult funded budget proposals for Child Welfare service users and Complex Special Care Needs in collaboration with the Team and external resources.Provide ongoing specialized adult case management supports and oversight to adult receiving funding through MCCSS living within Ontario.Liaise with Central Navigation regarding program/referral efficiencies including process development, Developmental Services Ontario (DSO) referrals, external communications and statistical reports.QualificationsPost-secondary Degree in Social Sciences, Social Work or related field or equivalent.Registered Social Worker designation is considered an asset.5 years of experience in Case Management experience.5 years of experience of direct support experience in a residential setting.Working knowledge of DSO and MCCSS’s Serious Occurrence reporting guidelines.Annual completion of a Criminal Background Check/Declaration, including Vulnerable Sector Screening (VSS).Ability to maintain positive and professional relationships with team members, community partners and other stakeholders.Compassionate and responsible supporter of people, their families, and community partners.Commitment to maximize resources and improve people’s quality of life.Valid G-Class driver’s license for the Province of Ontario, access to reliable transportation and have valid proof of automobile insurance with driving for business purposes and a minimum liability of two million dollars.About UsWe are proud to be located in Peterborough, Ontario, known traditionally as Nogojiwanong, an Anishinaabemowin word meaning place at the foot of the rapids.
