We are seeking an experienced and dynamic Manager, Primary Healthcare to join our leadership team. This role requires exceptional skills in relationship-building — with physicians, interprofessional staff and providers, community organizations, educational institutions, and non-traditional partners to address the social determinants of health of our community. The Manager co-designs and implements initiatives that improve patient experience, system integration, and population health outcomes for patients interfacing with the HFHT and supporting the broader system across the Greater Hamilton Health Network (GHHN) Primary Care Network. What You’ll Do Strengthen primary health care integration by building trust-based relationships with HFHT physicians, practitioners, and community partners. Lead team-based care expansion, co-location initiatives, and other Ontario Health or Primary Care Action Team (PCAT) initiatives, including project planning, documentation, and evaluation. Apply quality improvement methods to track progress, capture lessons learned and improve processes. Co-design initiatives with patients, providers, and community partners to ensure services are person-centred and equitable. Develop, monitor, and manage project budgets to ensure alignment with organizational goals and fiscal accountability. Support neighbourhood-based planning efforts, pairing with other managers to ensure consistency, shared learning, and sustainability. Providing a primary point of contact for feedback, needs, and problem-solving for the physicians and practices. Partner with the Physician Affairs team to ensure physician voices shape planning, policy, and decision-making. Collaborate with Clinical Managers to bridge interprofessional teams and physician practices, enabling coordinated and effective care delivery. Build and maintain partnerships with community organizations and non-traditional providers (housing, food security, cultural organizations, mental health services) to address the social determinants of health. Represent HFHT at regional and sub-regional planning tables, influencing integration strategies and system design. What You Bring Bachelor’s degree in a relevant discipline (e.g. health, social sciences, business administration, public, or related field), Master’s degree preferred. Minimum 5 years’ progressive leadership experience in healthcare, primary care, integrated healthcare environments, and/or working with physicians. Demonstrated experience bridging sectors, leading large organization‑wide or system‑level initiatives, or system integration. Proven relationship‑building skills across diverse stakeholders, including physicians, interprofessional teams, and community agencies. Strong skills in quality improvement, stakeholder engagement, and collaborative planning. Commitment to equity, diversity, and inclusion in all aspects of work. Why join the HFHT?
