Social Worker – Mental Health, FT (J1125-0250) Join us at North York General Hospital, one of Canada’s leading community academic hospitals. We offer a full‑time, 8‑hour day shift position in the Adult Mental Health – Day Hospital Program. Position Summary As a Social Worker in the Adult Mental Health Program – Day Hospital, you will be a fully integrated member of an interdependent multidisciplinary team. You will be responsible for providing individual, family, and group therapy using best practices, and for developing, implementing, and evaluating program activities. You will also coordinate community resources and support patients in crisis planning. Responsibilities Work in accordance with regulatory standards and professional practice guidelines, policies and procedures for North York General Hospital and the Ontario College of Social Workers. Demonstrate knowledge and experience in individual and group therapy modalities. Perform social work duties as required to meet the needs of the patient/client population. Maintain updated knowledge of community resources, identify gaps in service and respond to client and community needs. Offer group, individual, or family counselling where appropriate. Guide patients in the development of a crisis plan that supports their wellness and recovery during day‑hospital treatment and at discharge. Collaborate effectively as a member of the health‑care team and take initiative and leadership when appropriate. Participate in program planning, implementation, and evaluation. Develop and maintain effective linkages with hospital and community service providers. Participate in the education of staff, students, and the community. Qualifications Current registration with the Ontario College of Social Workers and Social Service Workers. Master of Social Work or equivalent. Extensive knowledge of local community mental health services is an asset. Clinical experience using CBT, DBT, and other relevant modalities is an asset. Excellent clinical skills. Membership in good standing or eligibility for membership in the Ontario College. Minimum 3‑5 years of mental health experience (outpatient or partial hospitalization setting is an asset). Membership with the Ontario Association of Social Workers strongly encouraged. Demonstrated competence to perform the Controlled Act of Psychotherapy.
