You’re looking in the right place.As a Care Coordinator, you will assess and determine patient care needs and eligibility, provide access and referrals to community services, and engage with patients, caregivers and other health care practitioners.Whether you work in our office, in the community, or a health care facility, you will play a lead role in providing connected, accessible, patient-centred care, and be supported by our collaborative team that includes over 8,000 regulated health care and other professionals.As a valued team member, your mission will be to help our patients be healthier at home, while you benefit from our supports for professional growth, personal wellness and work-life balance.What will you do?In collaboration with patients and their families, assess care needs, determine eligibility for services, and develop individual care and service plansLink patients with service providersCoordinate and monitor care plan deliveryEstablish a helping relationship with patients and their familiesBalance patient needs and choices with available resources, ensuring patients’ values and preferences are respectedWhat must you have?Membership, in good standing, with the applicable regulatory bodyCollege of Nurses of Ontario - (RNs ONLY)College of Physiotherapists of OntarioCollege of Occupational Therapists of OntarioOntario College of Social Workers and Social Service WorkersCollege of Audiologists and Speech Language Pathologists of OntarioThree (3) to five (5) years of recent experience in children\'s health with a focus on children and youth with medical complexity and high risk/high utilization conditionsUnderstanding of Ontario\'s provincial supports and strategies available to children with medical complexity and other conditions e.g. Complex Kids Ontario, Ministry of Health, Ministry of Children, Community and Social Services, and Ministry of EducationExperienced and knowledgeable regarding community services and resources for children and youth within Mississauga Halton GeographyClinical expertise providing patient and family-centred care, including critical thinking, ethical decision-making, and an ability to collaborate with a variety of different stakeholders including tertiary hospitals, community partner providers, primary and school staff, to ensure seamless delivery of care at home and participation at schoolDemonstrated leadership abilities to support inter-professional teams, including partners and providers within the circle of care, caregivers, parents, school staff, respite providers and colleaguesExcellent interpersonal, communication, assessment, problem-solving, and decision-making skillsEffective time management, prioritization and organizational skills, with the ability to work independently and co-operatively in a busy multidisciplinary environmentEstablished ability to accurately complete required documentation, reports and formsA valid driver’s licence and access to a reliable vehicleProficient in a Windows environmentWe have a mandatory COVID-19 vaccination policy.
