We are seeking an excellent communicator, critical thinker, lifelong learner and problem solver.As a Palliative Care Coordinator, competencies include clinical expertise in palliative care, patient and family-centred, ethical decision-making, collaboration with a variety of different stakeholders, and demonstrated leadership with inter-professional teams, including providers within the circle of care for individual patients and caregivers, and among the community with partners and colleagues. Experience in medication management and/or medication reconciliation and familiarity with medical diagnoses and disease trajectories are critical skillsets that will be a focus of recruitment.Whether you work in our office, in the community, or a health care facility – you will play a lead role in providing connected, accessible, patient-centred care – and be supported by our collaborative team that includes over 9,000 regulated health care and other professionals.As a valued team member, your mission will be to help our patients be healthier at home, while you benefit from our supports for professional growth, personal wellness and work-life balance.What you will do?Providing care coordination to patients with palliative needs, supporting patients with palliative needs to remain at home and in community through stable, transitional phases, and end of life.Development of individualized, collaborative care plans and care conferencingFacilitating communication and collaboration between the inter-professional care team for each patientHome visits and telephone communication with patients and caregivers; liaison with primary care providers and members of a patients circle of care.Assessing, planning, coordinating, implementing and reviewing patient needs and servicesProviding information to patients and referrals to alternate community resourcesResponding to inquiries and request for care in accordance with the patient’s needs; identifies risk factors and urgency for careIn collaboration with patients and their families, assess care needs, determine eligibility for services, and develop individual care and service plansLink patients with service providersCoordinate and monitor care plan deliveryEstablish a helping relationship with patients and their familiesBalance patient needs and choices with available resources, ensuring patients’ values and preferences are respectedWhat must you have?A registered health or social work professional including: registered nurse, physiotherapist, occupational therapist, speech language pathologist, or social worker.A member in good standing with their applicable regulatory body below:College of Nurses of Ontario (RNs ONLY at MH)College of Physiotherapists of OntarioCollege of Occupational Therapists of OntarioCollege of Audiologists and Speech Language Pathologists of OntarioOntario College of Social Workers and Social Services WorkersA University degree preferred. An equivalent combination of education and experience may be considered.Minimum two years recent experience in community health or a related field (acute, hospice, home and community care settings).
