Speech Language Pathologist - Community Stroke Rehabilitation - Part-Time Department: Rehabilitation Services Location: Kingston (Westbrook / Cataraqui Woods / Cedarwood) Under the supervision of the Program Manager, Allied Health & Clinical Services and working as a member of the outpatient Community Stroke Rehab team, the Speech-Language Pathologist is responsible for the provision of high quality rehabilitation to a patient or group of patients. The Brockville General Hospital Stroke Program supports and champions stroke best practices across the care continuum, contributing to the Brockville General Hospital’s mission of collaborating for exceptional care. The Community Stroke Rehabilitation Team is an integral component of the stroke continuum of care. Community Stroke Rehabilitation is person-centered, coordinated care provided by an interdisciplinary team with stroke specific expertise, who are informed by best practice guidelines. Person-centered care is delivered in the setting(s) that best meets the needs of the person with stroke, their family and caregivers. The team, in collaboration with other service providers, provide therapy, education and support to achieve functional recovery and optimize community reintegration. The Speech-Language Pathologist is a regulated health care professional responsible for providing patient care through all stages of assessment, treatment, consultation, and follow-up services to patients. They will work within their scope of practice and use the principles of client- and family-centered care to provide SLP services including assessment and treatment to optimize functional skills. The SLP will collaborate with community service providers to facilitate/promote independence and an improved quality of life and a return to activities that are important for the client and the caregiver. They provide client-specific consultation and education/training/in-services related to stroke rehabilitation for families, caregivers and service providers. As an expert in evidenced based stroke care, the SLP acts as a consultant and resource for other healthcare professionals concerning stroke education and management and works in collaboration with same to build capacity and expertise for stroke care. The Speech-Language Pathologist is expected to maintain competence and skills through participation in on-going continuing education and quality assurance activities. They will participate in-service and hospital-wide activities and programs that model and promote a safe and healthy patient-centred environment and that contribute to quality improvement and risk management initiatives. Requirements: Education: Masters degree from a recognized accredited university program in Speech-Language Pathology A member in good standing of the College of Audiologists and Speech-Language Pathologists of Ontario Aphasia Institute’s Supported Conversations for Adults with Aphasia (SCA™) preferred Certificate in Non-Violent Crisis Intervention training preferred Experience: Clinical expertise and experience in stroke and/or neurological rehabilitation populations Demonstrated knowledge of current Canadian Stroke Best Practices Experience in both hospital and community service delivery is an asset Knowledge of and recent clinical experience within the LLG communities and healthcare system preferred Skills and Abilities: Understanding of the role of regulated and unregulated healthcare professionals in the provision of stroke rehabilitation Demonstrated knowledge and clinical experience in neurological rehabilitation Working knowledge of and experience with theories and principles guiding adult learning and client self-management approachesProven flexibility and a high level of initiative Motivated team player with excellent organizational and planning, leadership, critical thinking, problem-solving, decision-making and time management skills Able to perform responsibilities with minimal supervision Excellent English verbal and written communication skills using different mediums, such as telephone, in-person and video Client, family and caregiver service-oriented, able to work effectively with diverse populations/groups Well-developed interpersonal skills with a demonstrated ability to establish positive relationships with clients, families, caregivers, colleagues and community services Demonstrated ability to prioritize and practice independently and collaboratively within an inter-professional team Excellent assessment skills and development of personalized client-centred treatment plans Able to support clients in their homes or in a community environment An understanding and appreciation of rural communities, including Indigenous perspectives, and the ability to navigate transitional services when needed Evidence of continued professional growth and/or professional development Proficiency with computer applications including Microsoft applications and working knowledge of or the ability to quickly comprehend the electronic medical record A valid Ontario driver’s license with access to reliable vehicle and auto insurance #J-18808-Ljbffr
