Our hiring practices aim to be inclusive, equitable and culturally safe.Detailed OverviewProvides speech language pathology services in accordance with the College of Speech and Hearing Health Professionals of BC (CSHBC) standards of practice, Fraser Health mission, values and SLP standards, and collaborates with the interprofessional care team to provide patient- and family-centered care. Responsibilities include assessment, diagnosis and prognosis in speech language and dysphagia, treatment planning and intervention, patient education, and participation in research, special projects and quality and safety activities.ResponsibilitiesProvide speech language assessment of the patient's communication, speech and swallow functions, including records review, patient interviews, goal setting, evidence-based assessment methods, collaboration with the health care team, and ongoing monitoring to support speech, language, voice and swallowing outcomes.Establish diagnosis and prognosis; develop evidence-based, outcome-focused intervention plans; set short- and long-term goals; select appropriate intervention models aligned with patient needs and goals.Implement interventions per treatment guidelines; educate patients and caregivers on self-management, health promotion and prevention.Participate in interprofessional care planning; update care plans and communicate issues affecting care, transfer, or discharge.Evaluate and document effectiveness of interventions; monitor patient responses and adjust plans to achieve optimal function.Identify appropriate referrals for speech language pathology services; screen intervention priorities and facilitate referrals to health care or community programs.Collaborate with patients and families in developing and modifying care plans; build therapeutic relationships while protecting privacy, dignity and patient autonomy.Maintain patient records in approved formats; document progress and test/treatment results; compile workload statistics as requested.Delegate components of treatment to speech and hearing assistants and provide training and supervision as needed; ensure quality of care during transfer of functions.Act as a resource to other professionals; provide information and in-services to the health care team and participate in meetings as a representative of SLP services.Participate in team/program activities to enhance care delivery and promote evidence-based practice; contribute to quality and safety improvements and research projects.Perform other related duties as assigned.QualificationsEducation and ExperienceCurrent registration with the College of Speech and Hearing Health Professionals of BC (CSHBC) including applicable college certification(s).Skills and AbilitiesKnowledge of SLP theory and practice within a patient- and family-centered model of care.Knowledge of CSHBC standards of practice and guidelines for clinical practitioners.Knowledge of dysphagia and communication assessment tests, measures, equipment and techniques.Broad knowledge of other health care professions and their roles in patient care.Effective verbal and written communication with patients, caregivers, and the interprofessional care team.Ability to teach and provide consultation to patients, caregivers, and other health professionals.Ability to work independently and collaboratively; prioritize workload and adapt to unexpected events.Basic knowledge of research methods; ability to operate related equipment and software.Physical ability to perform the duties of the position.About Fraser HealthFraser Health is the heart of health care for over two million people across Metro Vancouver and the Fraser Valley on the traditional, ancestral and unceded lands of the Coast Salish and Nlaka’pamux Nations, and is home to 32 First Nations within the Fraser Salish region.
