Community Disability Practitioner, Full-time Day Hours Base pay range: CA$22.04/hr - CA$25.99/hr Community Disability Practitioner Full-Time Day Hours:Monday - Friday 8:00am-3:30pm. Overview Role and Responsibilities Understand and practice all RHA Policies and Procedures, practices and operational protocols, including mission statements, ethical standards and guidelines, restrictive procedures, abuse and medical protocols, Alberta Accommodation Standards, Alberta Health Services standards, Municipal Fire Standards, and Creating Excellence Together (CET) Standards and Accreditation. Adhere to RHA Code of Ethics. Professional conduct and appearance befitting a representative of RHA, including attendance, punctuality and conflict resolution. Work effectively in a collaborative, team-oriented environment. Promote safe environments by regularly checking for and reporting any hazards that may have developed in the maintenance, design, and/or function of physical space. When hazards are identified, staff will ensure the safety of themselves and/or individuals to avoid the risk. Be awake, aware, and vigilant throughout duration of shifts, unless assigned to a sleep shift (and paid accordingly). Due to the nature of work supporting individuals with developmental and physical disabilities, all employees are required to possess a minimal standard level of physical capability. The ability to meet these physical job demands will ensure safety for the employee and the individuals served. All employees may be required to work in any program area during staffing shortages or emergencies or to better support the needs of the individuals or program. Specific responsibilities Understand and demonstrate a respect of individual rights and responsibilities, encourage choice and increased independence; review, implement and provide training and monitoring of Individualized Service Plans. Assist individuals with accessing the community, including transportation training and support. Work in the community to open doors and provide access/participation opportunities, and identify alternate avenues for generic services available to the individual. Communicate professionally with individuals, families, guardians, staff and community representatives; provide accurate information as required through written and verbal communication. Assist with orientation, information, support and feedback to new individuals and their family/guardian; assist with completion of checklists as required. Assist, support and encourage individuals to achieve personal goals through development of basic living and related skills, enhancing confidence, competence and participation within their social network. Assist and support individuals in establishing and maintaining shared, respectful relationships within the community, promoting meaningful participation in employment, volunteering, recreation, life skills and other settings. Provide transportation to individuals in personal vehicles where required by program/individual needs. Be aware of the individual’s environment; minimize risks and maximize participation; refer complex situations to more experienced resources when appropriate. Use effective communication methods and techniques (body language, non-verbal cues, behavior, active listening). Provide advocacy, personal and health care services for individuals in consultation with family/guardians. Implement individualized service plans for adults, youth and children in development, community support, behaviour management and/or personal care. Identify and maintain safety standards for individuals and staff according to policies and provincial standards. Promote a safe environment and ensure individuals are comfortable with their contributions in activities. Encourage and monitor good nutrition; implement dietary plans; monitor breaks; maintain cleanliness and sanitation. Model socially appropriate behaviors and help individuals build community connections. Ensure activities are age-appropriate in consultation with family/guardians. Maintain familiarity with module or activity resources and curriculums; assist with recording attendance and outcomes. Implement supervisor and professional recommendations and participate in program evaluations. Conduct program evaluations on service delivery within specific program areas; identify concerns and assess need for intervention. Understand use of technical aids (communication devices, adaptive equipment) and module aids (video, TV, projector). Stay informed of developments related to modules and individuals; provide feedback to instructors regarding modules and participant progress. Assist in addressing service gaps and communicate progress to supervisors; maintain organized, accurate data. If instructing, develop and use evaluation techniques, lesson plans, and store teaching resources to increase engagement; monitor participant engagement and coordinate with program staff to improve outcomes. Communicate effectively with program staff to address classroom concerns; provide tutoring on computer skills by request; conduct program evaluations on service delivery in classes. Operate within given financial resources for teaching resources and activity materials; provide direct care and services as scheduled and required. Maintain clear records and required reports; ensure data collection is professional. Participate in hiring processes; manage day program staff and quality individual programs; supervise regular/relief staff, practicum students and volunteers. Promote competence through counseling, coaching and role modeling; participate in team meetings and provide feedback for evaluations of practicum students, relief staff and volunteers. Assist with orientations for relief staff, volunteers and practicum students; provide feedback and answer questions.
