We are seeking an experienced Care Planner & Relationship Coordinator to join our team.Compensation & LocationBase Salary: $36,000 - $40,000 annuallyAdditional Earning Potential: Up to $8,000-$12,000 annually through client retention incentives and quarterly performance rewardsLocation: Serving Hamilton, Oakville, Burlington, and MississaugaScheduleFull-time, Monday to Friday (occasional evening/weekend flexibility required).Position OverviewWe seek a dynamic healthcare professional to develop business relationships, coordinate client services, and ensure quality care through PSWs with consistent supervision and training. This role involves significant fieldwork, spending 50-60% of the time across our service area.Key ResponsibilitiesDrive business growth through active networking and relationship-building with healthcare partners, including hospitals, clinics, retirement homes, and community organizations.Conduct regular networking visits, participate in healthcare forums, and represent the company at community events.Manage the process from receiving a referral to converting it into an active client – from initial contact to care plan implementation.Track leads, conduct assessments, present care solutions, and ensure a smooth transition into our service.Maintain high standards of care through comprehensive quality assurance: initial client assessments, development of personalized care plans, and regular quality assurance visits to client homes.Monitor service delivery, evaluate PSW performance, and ensure client satisfaction with regular feedback and prompt resolution of concerns.Document all visits, assessments, and interactions in detail.Staff management and development: participate in PSW recruitment and screening, conduct orientation sessions, implement ongoing training, and support the PSW team to ensure compliance with standards.Maintain our reputation for excellence in home healthcare through staff development and quality control.Extensive fieldwork throughout the Hamilton, Burlington, Oakville, and Mississauga regions, focusing on building community relationships and high-quality care delivery.Required QualificationsPSW Certificate is requiredFluent in English (written and verbal); other languages are an assetA healthcare-related degree or diploma is an assetMinimum 5 years of hands-on PSW experience, including 1-3 years in supervision/coordinationValid G driver's license with a clean driving recordReliable vehicle with proper insuranceExcellent communication and interpersonal skillsProficiency in healthcare documentation and Microsoft Office (Word, Excel)Scheduling software experience is an assetCriminal Background Check and Vulnerable Sector Screening requiredEssential Skills & QualitiesExcellent time management and organizational abilitiesStrong networking and relationship-building capabilitiesProfessional presence and presentation skillsAbility to work independently and manage multiple prioritiesComfortable with extensive daily travel throughout the service areaDetail-oriented with strong documentation skillsTravel RequirementsExtensive daily travel throughout the service area.
