Pay: $100,000 to 108,000 annually Queen Square Family Health Team (QSFHT) provides accessible, equitable, patient-centered primary care to its local community. A talented, interdisciplinary team works collaboratively with Queen Square Doctors, and together, we serve 35,000 patients annually. We offer a wide range of programs and services including diabetes care, seniors’ wellness, palliative care, mental health, maternal and children’s health, and gender affirming care. We are seeking a full time Clinical Pharmacist to support patient care delivery and lead the implementation of programs that advance safe, effective, and evidence-based medication use. The pharmacist will serve as a trusted resource to physicians and interprofessional team members, offering consultative services, education, and evidence-informed guidance that enhance both patient health outcomes and provider expertise. Key Responsibilities Prepare and optimize evidence-based, guideline-directed medication therapies and management plans in collaboration with family physicians and patients. Monitor and follow up on pharmacotherapeutic treatments and care plans to ensure safety and effectiveness. Address polypharmacy, identify and manage drug–drug and drug–disease interactions, and support the management of adverse drug reactions. Contribute to chronic disease management through patient and provider education. Respond to and resolve medication-related questions to inform patient care plans. Collaborate with interprofessional team members to improve medication management across a range of conditions. Share results of assessments and interventions with physicians and other team members. Provide physicians and providers with up-to-date drug information, clinical guidelines, and therapeutic recommendations to support evidence-based care. Support patients in developing self-management strategies that promote engagement and adherence. Develop, update, and maintain educational resources for both patients and providers. Lead or participate in individual and group education sessions for patients and providers. Design and deliver programs or workshops at times and locations that meet patient needs. Establish and update policies and standard procedures for safe medication management, including the integration of new medication information. Recommend improvements to the medication use process (e.g., handling of samples, administration practices, documentation) to enhance safety and efficiency. Facilitate referrals and support integration and communication between primary care, hospital, and home care services. Perform other duties as assigned by the Program Manager, Medical Director, or Chief Executive Officer. As a member of the Queen Square Family Health Team you will: Work with community and hospital partners to strengthen Queen Square’s program and advance patient care services. Contribute to program planning, implementation, and evaluation. Identify and be involved in quality improvement activities, including the collection of data for measurement, reporting, and evaluation. Participate in team-building and team meetings. Support and contribute to a healthy workplace that embraces lifelong learning, diversity, clinical teamwork, privacy, and adherence to professional standards of practice. Qualifications Minimum Bachelor’s Degree in Science (i.e. B.Sc. Pharm, PharmD or equivalent) Registration with the Ontario College of Pharmacists (Part A) required Community or Hospital Pharmacy residency, or equivalent clinical experience in either setting, is required. Preference will be given to candidates with experience working within a community- or hospital-based clinical team delivering related services Proof of professional liability insurance Knowledge and experience in pharmacotherapy, community health, public health sciences, and chronic disease management Experience working as part of an interdisciplinary team Comfortable with electronic documentation and use of technology in the workplace (Queen Square's EMR is Accuro EHR) CDE, BCGP certifications, or an interest in gaining experience in these areas is an asset Experience in chronic pain management and opioid deprescribing or tapering are an asset Skills Excellent written and oral communication skills Confidence to practice independently Excellent interpersonal and conflict resolution skills Excellent critical thinking skills and clinical decision-making Excellent organization skills and the ability to plan and execute program initiatives Proficient in Microsoft Windows, Microsoft Office, Microsoft Teams and Zoom Conferencing platforms and the ability to learn and adapt to new online tools as they become available See value and able to work effectively as a collaborative team member Queen Square Family Health Team offers a competitive salary and comprehensive benefits package, including medical and dental benefits, and is a participating employer of the Healthcare of Ontario Pension Plan (HOOPP).
