Position Summary:

The Associate Director of Care will ensure residents receive excellent care and services while ensuring operational excellence for each resident home area. The Associate Director of Care will lead the nursing team with all care activities, ensure accurate completion of all documentation, and be a key liaison for all residents and family members.

Key Responsibilities:

Resident Care & Services:

  • Plan, direct, develop, co-ordinate, implement and evaluate resident care services; oversee clinical care processes to ensure that care is delivered according to individual plans of care for residents.
  • Liaise with hospitals, doctors, external providers, and available interdisciplinary services to ensure the most effective provision of care delivery in the home.
  • Lead the implementation and monitoring of required clinical programs, i.e., Falls Prevention, Skin and Wound Care, Continence Care, Pain Management.
  • Participate actively in committees according to the terms of reference and areas of responsibility.
  • Uphold accreditation standards and ensure that program and practice standards are consistent with professional guidelines; ensure all team members are following best practices, legislation, and professional guidelines.
  • Oversee the development, implementation, review, and evaluation of resident care plans; ensure all RAI-MDS assessments are completed on time, all care conferences are held, and information is captured for discussion, ensuring all data is ready for submission in partnership with the RAI coordinator and CIHI.

Operational Experience:

  • Ensure quality care and a safe, effective, and efficient work environment.
  • Assume responsibilities of the Director of Care as required in their absence.
  • Be knowledgeable of the Resident Bill of Rights, LTCHA, and associated regulations, and always promote a resident-first philosophy.
  • Ensure resident and family satisfaction and promote continuous quality improvement initiatives.
  • Coordinate and ensure care processes for the RAI MDS Program, Nursing Rehab, and Restorative Care Program are implemented, including evaluation outcomes and monitoring of performance metrics.
  • Lead the coordination and implementation of best practice and risk management initiatives related to resident care.
  • Conduct regular audits and in-services to ensure continuous quality improvement in conjunction with other leaders and departments.
  • Collaborate continuously with the interdisciplinary team to ensure a resident-centered care approach to all service and care offerings; maintain regular communication with residents and loved ones as well as Residents Council as needed.

Human Resources:

  • With support from the Director of Care, hire, direct, supervise, evaluate, and performance manage personal support workers and registered team members.
  • Monitor performance, conduct performance appraisals, and work with the Director of Care to resolve performance concerns.
  • Lead, coach, and mentor team members; support continued learning opportunities and foster a positive working environment; act as the department lead for staff training, education, and professional development.
  • Collaborate with the Director of Care in the implementation and evaluation of the nursing department initiatives and functions.
  • Work with the Director of Care and other leaders as needed to conduct investigations and produce associated findings reports.
  • Oversee human resources functions including scheduling, recruitment/retention, onboarding and orientation, health and safety, return to work plans, and performance management (progressive discipline, absenteeism management, etc.).

Customer Service:

  • Model and foster an environment of positive customer service in all aspects of care.
  • Carry oneself in a professional and personable manner at all times, ensuring appropriate conversation and interactions with all stakeholders.
  • Ensure residents and/or families are kept up to date on health status, changes, and other pertinent information from HMLTC.
  • Facilitate interdisciplinary care conferences with the assistance of the Social Worker(s), with special attention to resident attendance.
  • Develop strong relationships with residents/families, becoming a trusted member of the circle of care; ensure professional relationships are fostered with all team members and residents/families.
  • Follow up on all concerns/complaints in a timely and professional manner while ensuring all policies are followed.

Qualifications:

  • Bachelor’s degree in Nursing or related science discipline.
  • Master's in Nursing or Healthcare is an asset.
  • Current Registration as a RN with the College of Nurses Ontario.
  • One year of experience as an RN in long-term care is required.
  • One year of experience in a supervisory role in long-term care is preferred.
  • Three years of experience as an RN in a supervisory or managerial role in healthcare is required.
  • Enhanced knowledge of LTCHA, Ont Reg 79/10, Residents Bill of Rights & RAI-MDS.
  • Demonstrated leadership and communication skills.
  • Possession of strong motivational, team-building, and time management skills.
