About the role

The Métis Healthy Babies Healthy Children (MHBHC) program is a prevention and early intervention strategy of services for families with children prenatal to 6 years of age. The heart of the MHBHC programming is preparation for parenting (preconception component), getting ready for the birth of the baby (prenatal care) and taking care of the baby (postnatal care). Client based services are services provided to an individual where significant contact and focused assistance is provided to the individual and/or on behalf of the individual.

What you will be doing

  • Ensure that Métis Healthy Babies Healthy Children program is delivered in the spirit and philosophy of the Métis Nation of Ontario’s Statement of Prime Purpose
  • Provide prevention and early intervention strategies of services for families with children prenatal to 6 years of age
  • Provide support to families preparing for parenting (preconception component), getting ready for the birth of the baby (prenatal care) and taking care of the baby (postnatal care).
  • Support client driven needs utilizing case management techniques such as establishing a trusting relationship; case noting, advocacy, referrals, development of personal wellness plans, peer support, follow-up, linkages, home visits, and interventions as necessary (baby weight tracking and child development)
  • Provide education/informing/learning activities related to prenatal health, parenting, nutrition, family plan, and activities such as healthy sexuality
  • Work with health and social agencies by way of organizing and facilitating community events to enhance client and community knowledge, and improve client access
  • Record client services in the Online Health and Access Tracking System (OHATS).
  • Maintain confidentiality at all times, and adhere to privacy legislation
  • Financial reporting specific to your program site and budget
  • Reflect Métis cultural approaches as a part of the activities and services
  • Participate in on-going professional development activities as time and financial resources permit
  • Attend training and meetings outside Métis Nation of Ontario working hours
  • Fulfill all other responsibilities as determined by management of the Community Wellbeing Branch

Qualifications

  • Strong desire to act in the service of the Métis Citizens, to advance and uphold rights and culture.

Mandatory qualifications:

  • Minimal education of a 2-year diploma in a social services related field
  • 1 or more years’ experience in client case management experience
  • 1 or more years’ experience working with ages 0 to 6
  • Proficiency with computer programs (Outlook, Excel, Work and PowerPoint)

Requirements:

  • Knowledge of the Métis Nation of Ontario and programs offered
  • Knowledge of Indigenous programs and services
  • Ability to work as part of a team and independently with minimal supervision
  • Well-developed administrative, organizational and communication skills in order to meet deadlines
  • Ability to lift 11.5kg when required
  • Bilingualism would be considered an asset
