As an integral team member of the Nephrology Program, there is an opportunity to participate within the Program by utilizing a chronic disease prevention and management (CDPM) and self-management approach and philosophy, as the Program continues to achieve the deliverables under the Ontario Renal Network, and strives to achieve the targets of the Ontario Renal Plan III:

  • Empower and support patients to be active in their care
  • Integrate patient care throughout the kidney care journey
  • Improve patient access to kidney care

Right now we’re looking for a Temporary Full-time Clinical Home Dialysis Coordinator to join our team within the Nephrology Program.

Using a collaborative and consultative approach, the Home Dialysis Coordinator, will assist in providing program/service leadership to create an environment for innovation and excellence in patient focused care. This position is accountable to the Director Nephrology Program. This position will take the lead in a comprehensive approach with all patients starting dialysis by liaising the patient/family with the Nephrologist and Interdisciplinary team with the purpose of providing assessments, education, treatment-options/shared decision making, along with support and follow up while encouraging independent dialysis modalities. The Home Dialysis Coordinator is also accountable to the Ontario Renal Network (ORN) through the Regional Director, and responsible for ORN Home Dialysis assessment and data collection for the program.

Hours of work: Monday to Friday, some flexibility is required.

Location: Wilson and Church Sites

Responsibilities:

  • Providing high quality, effective, efficient and safe patient services through human, environmental, and financial resources management.
  • Demonstrating an empathetic, resourceful and direct approach to successfully assist patients through their renal journey and in their adjustment to starting dialysis.
  • As an integral team member of the Nephrology Program, there is an opportunity to assist the Program by utilizing a chronic disease prevention and management (CDPM) and self-management approach and philosophy, as the Program continues to achieve the deliverables under the Ontario Renal Network, and strives to achieve the targets of the Ontario Renal Plan II: * Empower and support patients to be active in their care * Integrate patient care throughout the kidney care journey * Improve patient access to kidney care
  • With support from the Regional Director and/or delegate, lead a team to implement and evaluate local quality improvement initiatives to promote and increase the uptake of Home Dialysis modalities
  • Identify rationale for modality type among prevalent patients and facilitate accurate reporting of data within the ORRS tracker tool
  • Collaborate with peers and adapt proven quality improvement change concepts to drive local improvements towards stated targets
  • Report back to the ORN on quality improvement initiatives implemented and related progress
  • Providing leadership and consultation to staff in the development, implementation and evaluation of related projects and initiatives for Independent Dialysis Program (Home Dialysis).
  • Acting as an expert resource to the clinical members of the Dialysis Program.

  • In collaboration with the Body Access Coordinator, coordinating pre and post body access creation, care and transfer of patients to the appropriate unit
  • Assisting the Clinical Practice Leader (CPL) with the development of staff to achieve advanced clinical practice standards for the Multi-Care Kidney Clinic, Transition Care Unit and Home Dialysis Program.
  • Participating in research projects, data/statistics collection and reporting to appropriate stakeholders.

 Requirements:

  • Registered and in good standing with the College of Nurses of Ontario
  • Baccalaureate degree, Master’s preferred
  • Valid CNeph Certification Nephrology Certificate – (all courses completed or commitment to complete within an agreed upon timeframe)
  • BLS Provider certified
  • Minimum of 3 years previous clinical experience in Nephrology nursing, with knowledge of all components of independent dialysis with emphasis in home dialysis therapies; home hemodialysis and peritoneal dialysis.
  • Experience in all aspects of body access including peritoneal dialysis catheters and vascular access/fistula/graft for hemodialysis required
  • Experience with ORN data collection (ID trackers/VE assessment) preferred
  • Demonstrated computer skills using Microsoft office (Word, Excel, Power Point, Outlook, Access) and Meditech
  • Demonstrated ability in adult teaching, knowledge of learning principles and assessing readiness to learn
  • Demonstrated leadership skills in program planning and development
  • Experience in developing/implementing patient algorithms and care paths
  • Demonstrated excellent interpersonal and communication skills with the ability to work with a multidisciplinary team including other health team members, patient/family
  • Experience in developing and leading Continuous Quality Improvement programs
  • Demonstrates being patient and consumer focused; makes a commitment to meet or exceed the needs of all internal and external customers
  • Strong communication and interpersonal skills; listens for clarity and meaning, and to speak in an honest and straightforward manner to ensure mutual understanding in a conversation
  • Demonstrated excellent patient assessment and organizational skills
  • Ability to be flexible and effectively work in busy environment with changing priorities and deadlines
  • Demonstrated initiative in improving work skills and processes
  • Satisfactory employment and attendance record
  • Valid driver’s license required and access to reliable transportation for travel between all HRH sites and participate in home visits if needed
