We are collaborating with Northwestern Medicine and this strategic relationship will help accelerate this high-quality transplant program. This initiative reflects BayCare’s continued focus on advancing complex, lifesaving care while strengthening our role as a leading academic health system in West Central Florida. This Transplant Licensed Social Worker will work directly with transplant patients at St Joseph's Hospital in Tampa, FL. The Transplant Licensed Social Worker Responsibilities Include

Act as member of collaborative practice model that includes patients, nurses, clinical resource coordinators, physicians, and other practitioners, caregivers, and the community Psychosocial Assessments:

Conduct thorough psychosocial evaluations to assess a patient's readiness for transplant surgery, recovery and rehabilitation Evaluating social support systems, family stability, adherence to medical advice, lifestyle behaviors and understanding of transplant risks and benefits. Identify potential barriers to transplant eligibility and help develop individual care plans.

Post-Transplant Support:

Provide ongoing counseling and resources to promote adherence to medication regimens, lifestyle adjustment and emotional well-being. Provide support to reduce anxiety, depression and potential substance misuse, improving long-term outcomes.

Quality Improvement Duties:

Develop clinical protocols Implement quality improvement initiatives

Required FL LCSW (Social Work) Education

Required Masters in Social Work Experience

Required 2 years' experience as Licensed Social Worker specific to Transplant Services Benefits
