Description

Reporting directly to the Director of Health and Social Development, The Case Manager will delivery and provide assistance to the First Nations communities that do not have Jordan's Principle agreement. Working with families ensuring resources are utilized and address gaps in services.

Responsibilities

Case management and outreach duties; service referrals; supporting families in self-advocacy skills. Collaborate with Community, internal staff and external professionals, organizations, and community members to identify services in the area. Keep an updated inventory of the services available for each community for children and youth. Keep up to date Data on the children and youth accessing Jordan’s Principle Respite services. Strong communication skills, ensuring there are no gap in services to clients and there is a strong collaboration to meet families needs. Use critical thinking, strength based, trauma informed approach when making choices that can impact families. Be mindful of the privilege and power of choices and be mindful of the short and long term outcomes. Protect the integrity and work culture of Dene People. Other duties as assigned.

Qualifications

An undergraduate degree or diploma in a Health & Social Services related field (i.e. family, human and/or social services, child and youth care, educational or behavioral intervention) or equivalent education and experience would be considered.
