About the Role

We are seeking a highly skilled and compassionate Mental Health Clinician to join our team at Fraser Health. As a key member of our Community Liaison Program, you will play a vital role in providing intensive individual and group therapy, liaison services, and care coordination support to individuals and families affected by mental illness and substance use disorders.

Key Responsibilities
  • Client Assessment and Treatment: Conduct initial screenings, assessments, and develop treatment plans in collaboration with clients, families, and interdisciplinary teams.
  • Intensive Therapy and Support: Provide individual and group therapy, counseling, and support services to clients, focusing on recovery, rehabilitation, and reducing barriers to care.
  • Liaison and Coordination: Establish and maintain relationships with community resources, service providers, and healthcare professionals to ensure seamless care coordination and access to services.
  • Case Management and Advocacy: Identify client needs, prioritize care, and advocate for clients to access necessary services, including emergency interventions and hospital admissions.
  • Community Engagement and Education: Facilitate client and family-centered care through a trauma-informed approach, promoting self-determination, independence, and knowledge exchange.
  • Quality Improvement and Collaboration: Participate in team planning, meetings, and local committees to promote quality improvement, streamline practices, and ensure optimal support for clients, families, and caregivers.
Requirements
  • Education: Master's degree in Social Work, Clinical Counselling, or a relevant health-related discipline.
  • Experience: At least two years of recent, related experience in a community or hospital psychiatric setting, or in a rapid response crisis stabilization function.
  • Skills: Excellent communication, assessment, and treatment planning skills; ability to work collaboratively with interdisciplinary teams and community resources.
