Care Coordinator

We are seeking a skilled Care Coordinator to join our team at Home and Community Care Support Services. As a Care Coordinator, you will play a vital role in assessing and determining patient care needs, providing access to community services, and engaging with patients, caregivers, and healthcare practitioners.
Key Responsibilities- Assess patient care needs and determine eligibility for services
- Provide access to community services and resources
- Engage with patients, caregivers, and healthcare practitioners to develop individual care plans
- Coordinate and monitor care plan delivery
- Establish a helping relationship with patients and their families
- Membership in good standing with the applicable regulatory body (e.g. College of Nurses of Ontario, College of Physiotherapists of Ontario, etc.)
- 2+ years of recent experience in community health or a related field
- Knowledge of the healthcare delivery system and community resources
- Excellent interpersonal, communication, assessment, problem-solving, and decision-making skills
- Effective time management, prioritization, and organizational skills
- Ability to accurately complete required documentation, reports, and forms
- Valid driver's license and access to a reliable vehicle
- Proficient in a Windows environment
- Experience working with diverse patient groups (e.g. multicultural, homeless, palliative, acquired brain injury, mental health, geriatrics, pediatrics)
- Case management experience or recent related community experience
- Ability to speak French or another second language
