Job Description: The Patient Care Coordinator follows assigned clients from admission to discharge providing case management through assessment, coordination, implementation, monitoring and evaluation of health related services to clients. Quality care and cost effective outcomes are promoted with the client's expressed health goals and general well-being at the forefront of the care plan. The Patient Care Coordinator maintains a collaborative client-centered approach that is continuous across provider and agency.

Education

  • Bachelor of Social Work

Licenses

  • Valid Class 5 driver's license

Other Education and Training

  • Bachelor of Science in Nursing is acceptable
