Seeking a Patient Care Coordinator to provide case management services to clients, promoting quality care and cost-effective outcomes.

Job Description

The Patient Care Coordinator follows assigned clients from admission to discharge, providing case management through assessment, coordination, implementation, monitoring and evaluation of health-related services.

  • Assess client needs and develop care plans to promote quality care and cost-effective outcomes.
  • Coordinate with healthcare providers and agencies to ensure seamless care delivery.
  • Monitor and evaluate client progress, making adjustments to care plans as needed.

Key Responsibilities:

  • Communication with healthcare providers and agencies
  • Monitoring and evaluation of client progress

Requirements:

  • Bachelor of Social Work (BSW) or Bachelor of Nursing
  • Valid Class 5 driver's license
  • Registered/eligible for registration with the appropriate licensing body
