Patient Care Manager (PCM) Rehab & Palliative Care (Job ID: 4900)

POSITION SUMMARY:

The Patient Care Manager is accountable for the day-to-day operations and utilization of resources for the patient care units by providing collaborative leadership, mentorship, education and communication within an inter-professional team to achieve measurable outcomes and performance targets with an emphasis on patient, family and staff satisfaction and safety. This position is responsible for the Palliative Care programs at St. Joseph's Health Centre and St. Michael's Hospital.

MAJOR RESPONSIBILITIES:

  • Participates and collaborates in planning, developing and delivery of programs and services; accountable for targets and outcomes;
  • Provides leadership to the Inter-professional team in order to ensure the holistic needs and preferences of individual patients are assessed and guide care planning;
  • Works closely with the Senior Clinical Program Director, Rehabilitation and Palliative Care to ensure quality programming, fiscal accountability, achievement of metrics to targets and program evolution to meet patient care demands;
  • Maintains direct communication and collaboration with patients and families to ensure participation in and satisfaction with care;
  • Liaises with external referral and care partners to ensure the admission, re-admission, consult and discharge process is well managed;
  • Maintains a positive working relationship with the Inter-professional team and develops a strong rapport with leader colleagues across the organization;
  • Ensures that an integrated, effective plan of care is developed, implemented and evaluated for all patients within the programs they oversee.

QUALIFICATIONS:

  • Clinical background with current registration in good standing with a recognized professional college;
  • Completion of a Master's Degree in Nursing, or equivalent;
  • Current certificate of licensure/ registration with the College of Nurses of Ontario or appropriate health discipline regulatory body;
  • Minimum 5 years' experience as an inter-professional team leader and/or manager in a similar health care setting;
  • Familiar with program management;
  • Clinical knowledge/experience in a health care setting providing care/service for older adults;
  • Experience working in and managing palliative care programs preferred
  • Experience with Alternative Level of Care and patient flow required
  • Experience in managing within unionized environments
  • Demonstrated ability to persuade and positively influence others;
  • Excellent communication skills and computer skills;
  • Proven ability to lead projects to full completion;
  • Knowledge and understanding of LEAN methodology an asset;
  • Very good attendance record;
  • Good performance record.
