Key Responsibilities- Conduct comprehensive assessments of patients to identify physical therapy needs and develop personalized treatment plans.
- Provide evidence-based physical therapy interventions to maintain or improve physical functioning in patients.
- Collaborate with inter-professional teams to provide patient-centered care and achieve optimal outcomes.
- Communicate effectively with patients, families, and healthcare professionals to ensure seamless care transitions.
- Develop and implement creative and comprehensive care plans based on best practices and evidence-based standards.
- Consult with internal and external stakeholders to facilitate smooth patient transitions and optimize care delivery.
- Participate in quality improvement initiatives and contribute to the development of best practices in physical therapy.
- Master's degree or equivalent in Physical Therapy.
- Current registration with the College of Physiotherapists of Ontario.
- Membership in the Canadian Physiotherapy Association (CPA) is preferred.
- Basic Cardiac Life Support (C Level) certification.
- Minimum 2 years of clinical experience in neuro rehabilitation, with a focus on ABI/dementia population.
- Excellent interpersonal and communication skills.
- Strong customer service focus and problem-solving skills.
