About the Opportunity

We are seeking a highly skilled and compassionate Patient Access Navigator to join our team at Nova Scotia Health Authority. As a key member of our Mental Health and Addictions team, you will play a critical role in facilitating access to mental health and addiction services for individuals in need.

The Patient Access Navigator will be responsible for:

  • Providing telephone access to mental health and addiction services
  • Utilizing screening tools and risk assessments to determine the level of care required
  • Facilitating referrals to mental health and addiction services
  • Providing brief supportive counseling and psychoeducation to individuals
  • Building relationships with community resources and mental health and addiction teams

The ideal candidate will have a strong background in social work, with experience working with individuals with mental health and addiction concerns. You will possess excellent communication and interpersonal skills, with the ability to work effectively with diverse populations.

About You

We are looking for a candidate who possesses the following qualifications and skills:

  • Bachelor's degree in Social Work
  • Registration with Nova Scotia College of Social Workers
  • Three years of experience working in mental health and addiction services
  • Excellent communication and interpersonal skills
  • Ability to work effectively with diverse populations
  • Knowledge of harm reduction strategies and stages of change theory
  • Experience utilizing screening and triaging tools

Hours
  • Long assignment full-time position; 75 hours biweekly
  • Approximately 12 months
