Clinical Pharmacist We are seeking a highly skilled Clinical Pharmacist to join our team at St. Paul's and Mount Saint Joseph Hospitals in Vancouver, BC. As a Clinical Pharmacist, you will play a vital role in providing clinical pharmacy services to our patients, including individualized pharmacotherapy, patient medication counselling, and consultation with physicians and other healthcare providers. Job Summary * Provide clinical pharmacy services to patients, including individualized pharmacotherapy and patient medication counselling * Consult with physicians and other healthcare providers to develop and discuss therapeutic plans * Assist in the development of drug therapy guidelines for individual disease conditions * Participate in medication use evaluations, quality improvement initiatives, and clinical drug trials * Provide instruction and/or supervision to pharmacy students and residents * Perform drug distribution duties, including interpreting medication orders/prescriptions and dispensing medications Responsibilities * Consult with physicians, nurses, and other healthcare providers to develop and discuss therapeutic plans * Individualize patient medication therapy by performing activities such as referencing standard literature/publications and patient demographic information * Provide patient medication counselling regarding drug therapy * Assist the healthcare team in developing drug therapy guidelines for individual disease conditions * Participate in ward rounds, program team meetings, and provision of pharmacy services at the unit level * Act as department resource person in the assigned area of responsibility * Contribute items such as case studies and review articles to professional journals * Provide instruction and/or supervision to pharmacy students and residents * Participate in quality improvement activities * Perform drug distribution duties, including interpreting medication orders/prescriptions and dispensing medications Qualifications * Current registration as a Pharmacist with the College of Pharmacists of British Columbia * Completion of an accredited Hospital Pharmacy Residency Program or an equivalent combination of education, training, and experience * Broad knowledge of organization policies, procedures, and standards of care * Knowledge of evidence-based clinical pharmacy practice * Knowledge of medication use evaluation * Knowledge of research processes and methodology * Ability to apply clinical pharmacy knowledge and make decisions with a minimum of supervision * Ability to communicate effectively both verbally and in writing * Ability to establish and maintain effective working relationships with other healthcare practitioners, as well as patients, family members, and the public * Ability to organize and prioritize work * Ability to operate related equipment * Physical ability to carry out the duties of the position About Fraser Health Fraser Health is a leading healthcare provider in Metro Vancouver and the Fraser Valley, serving over two million people.
