As a Clinical Pharmacist, you will work collaboratively with physicians, nurses, and other healthcare providers to develop and implement therapeutic plans for our patients. You will also provide individualized pharmacotherapy, counseling patients about their medication therapy, and assist in the development of drug therapy guidelines for individual disease conditions. **Responsibilities** • Consult with physicians, nurses, and other healthcare providers to develop and discuss therapeutic plans for each patient • Individualize patient medication therapy by referencing standard literature, reviewing patient charts and laboratory test results, and modifying or developing therapeutic plans as needed • Provide patient medication counseling regarding drug therapy, reviewing reference literature, summarizing information, and meeting with patients and their families to provide and explain information and plans • Assist the healthcare team in developing drug therapy guidelines for individual disease conditions by reviewing current literature, compiling and analyzing information, and providing suggestions and recommendations • Participate in ward rounds, program team meetings, and provision of pharmacy services at the unit level to enhance patient care • Act as department resource person in the assigned area of responsibility, providing detailed drug information on issues related to drug therapy • Contribute items such as case studies and review articles to professional journals by identifying topics, gathering and analyzing data, reviewing current literature, and writing and submitting articles for publication • Provide instruction and/or supervision to pharmacy students and residents by preparing educational material, demonstrating procedures, lecturing, and supervising during patient care activities • Participate in quality improvement activities through methods such as reviewing current practices, performing quality assurance audits, identifying actual or potential problems, and recommending changes • Perform drug distribution duties, including interpreting medication orders/prescriptions, dispensing medications using a computerized medication information system, and reviewing prescriptions for therapeutic suitability **Qualifications** • Current registration as a Pharmacist with the College of Pharmacists of British Columbia • Completion of an accredited Hospital Pharmacy Residency Program or an equivalent combination of education, training, and experience • Broad knowledge of organization policies, procedures, and standards of care • Knowledge of evidence-based clinical pharmacy practice, medication use evaluation, research processes and methodology, and drug distribution processes in a hospital setting • Ability to apply clinical pharmacy knowledge and make decisions with a minimum of supervision • Ability to communicate effectively both verbally and in writing • Ability to establish and maintain effective working relationships with other healthcare practitioners, patients, family members, and the public • Ability to organize and prioritize work • Ability to operate related equipment • Physical ability to carry out the duties of the position **About Fraser Health** Fraser Health is a leading healthcare provider in Metro Vancouver and the Fraser Valley, serving over two million people.
