Registered Nurse (Nurse Practitioner) sought to provide Primary Health Care services to assigned populations in urban and rural communities.

The ideal candidate will have experience in community needs assessment, health promotion, and disease prevention, with a strong background in nursing and a Master of Nursing - Nurse Practitioner degree.

The successful candidate will be able to work independently and as part of a multi-disciplinary team, with excellent communication and interpersonal skills.

Job Description

In collaboration with Primary Health Care Teams, physicians, and health care provider team members, the Registered Nurse (Nurse Practitioner) will provide Primary Health Care services to assigned populations.

  • Health assessment, health promotion, and education
  • Disease and injury prevention and assessment
  • Treatment interventions
  • Collaboration with, and referral to, other health care providers

The position may include first on-call support and participation in Lean, patient safety, community development, clinical practice redesign, quality improvement, and evaluation.

  • Travel to sites within the service area
  • Development and delivery of formal education programs
  • Ability to work with diverse cultures, values, and traditions

  • Experience as RN (NP) in a Primary Health Care setting
  • Master of Nursing - Nurse Practitioner degree
  • Valid Class 5 driver's license
