About the RoleThe Community Health Nurse assesses, plans, delivers, and evaluates the care needs of community members in community settings, emphasizing the independence and health promotion of the client.
The primary areas of responsibility of the Community Health Nurse include:
- Health promotion and education
- Community health development
- Assessing specific care needs and educating individuals or groups
- Providing health education and promotion services to individuals and target groups
- Ensuring all equipment related to client care is hygienic and in safe working order
- Planning, organizing, implementing, and evaluating community health services
- Analyzing community health indicators
- Providing treatment, support, and follow-up for compliance
- Teaching treatment procedures and care techniques to clients and families
- Developing action plans for individuals, families, schools, and communities to deal with health emergencies and pandemic responses
- Selecting services and activities for positive health changes
- Advocating and referring community members to appropriate agencies, medical providers, and programs or services
- Managing and coordinating client visits with physicians, hospitals, physiotherapy, occupational therapy, dentists, optometrists, and audiology
- Liaising with physicians, hospitals, teachers, social workers, and other community services agencies
- Maintaining accurate recording and reporting systems
- Identifying health issues and needs through analysis of area, population, health, and other relevant data
- Providing services and establishing a means to measure effective results
- Coordinating Public Health Nursing services with community and services agencies
The Community Health Nurse will also be responsible for:
- Developing community health work plans
- Developing health promotional materials and/or displays as a team member
- Community Health reporting, as required
- Conducting home/office visits
- Attending professional development training to provide current care, techniques, and practices while delivering Community Health Care
- Attending community meetings and workshops that relate to the CHN position
- Promoting traditional and cultural health practices
- Preparing course material and presenting individual or group sessions
- Ensuring that care is provided according to all relevant policies, procedures, and regulations
- Coordinating case management when necessary
- Providing information to other health care professionals, as required
- Ordering supplies for Community Health program and maintaining accurate inventory and records
- Developing and maintaining a current, accurate, confidential client reporting and filing system
- Maintaining the integrity of vaccinations
- Biohazard management
- Attending meetings
- Making recommendations for changes and improvements to the program, as required
- Reporting any concerns to the Director of Health and Social Development
We are seeking a qualified Community Health Nurse who:
- Has a Bachelor's degree in nursing (RN) from a recognized university
- Is currently registered with the BC College of Nursing Professionals (BCCNP)
- Has a current First Aid/CPR certificate with AED
- Has immunization certification and knowledge/training in managing communicable disease within a community setting
- Has a minimum of two years of experience working in a community setting
- Has experience working with First Nations communities
- Has strong team/client leadership, communication, problem-solving, and teamwork skills
- Has experience with program planning, delivery, and facilitation
