We are seeking a skilled and compassionate Community Health Nurse to join our team at Saint Elizabeth Health Care. As a Community Health Nurse, you will play a vital role in providing high-quality patient care and support to individuals in their homes and communities.
Key Responsibilities- Conduct comprehensive client assessments to identify individualized care needs and develop personalized care plans.
- Implement and evaluate care plans, utilizing evidence-based practices and client-centered approaches.
- Collaborate with interdisciplinary teams to ensure seamless care transitions and coordination.
- Provide education and support to clients and their families to promote health and wellness.
- Current registration with the College of Nurses of Ontario.
- Canadian Nurses Association (CNA) specialty certification an asset.
- Enthusiasm and love of community care, with a passion for evidence-informed practice and excellence in care.
- A current CPR certificate is required.
- A valid driver's license and vehicle (travel required).
- Updated immunization records, clear criminal background check, and Vulnerable sector check are required upon hire.
- A willingness to attain required Saint Elizabeth certifications.
- Availability to work days, evenings, nights, and every other weekend.
