About the Job

Fraser Health is seeking a Registered Occupational Therapist to join our Home Health Community Rehabilitation team in Surrey, B.C.

Key Responsibilities
  • Assess clients' cognitive, perceptual, self-care, and physical status to develop personalized care plans.
  • Collaborate with clients, caregivers, and healthcare professionals to formulate treatment goals and implement care plans.
  • Prescribe and recommend specialized and adaptive aids and equipment to clients for activities of daily living.
  • Coordinate rehabilitative services with clients and other healthcare providers to ensure seamless care.
  • Act as a client advocate by providing information and liaising with funding agencies and community resources.
  • Participate in quality improvement and risk management activities to ensure high standards of care and client safety.
  • Maintain patient records and reports statistics to accepted standards.
Qualifications
  • Current full registration with the College of Health and Care Professionals of BC and eligible for membership in the Canadian Association of Occupational Therapists.
  • Two (2) years' recent, related clinical experience in a community or home health setting, or an equivalent combination of education, training, and experience.
  • Valid BC Driver's License and access to a personal vehicle for business-related purposes.
