Key Responsibilities
  • Assess, plan, implement, and evaluate patient care needs in collaboration with the interdisciplinary team.
  • Develop and maintain strong relationships with patients, families, and healthcare professionals to ensure seamless care delivery.
  • Collaborate with various health disciplines and family members to achieve patient goals.
  • Stay up-to-date with the latest nursing practices and evidence-based care standards.
  • Participate in quality improvement initiatives to enhance patient outcomes and experience.
Requirements
  • Diploma in Nursing or a Bachelor Degree in Nursing.
  • Active registration with the Nurses Association of New Brunswick.
  • Minimum of two years of specialized nursing experience in family practice, surgical, palliative care, or oncology.
  • Knowledge of the target population of Extra-Mural Services and available community services.
  • Valid New Brunswick Class 5 Driver's License and good driving record.
  • Clean criminal record check and current CPR Level C certification.
  • Excellent communication, leadership, and interpersonal skills.
