**Job Summary:**
We are looking for a detail-oriented and patient-focused Registered Practical Nurse (RPN) with administration experience to coordinate patient care and support our specialist physician practices.

**The Role:**
As a Patient Care Coordinator, you will be part of a team of 10 members working together to facilitate timely and high-quality care for patients attending KMH Clinics. Your responsibilities will include:
• Providing guidance, support, education, and advice to patients
• Serving as a liaison between the multidisciplinary team and external service providers to ensure high standards of quality and optimal management of patient care outcomes
• Coordinating in-clinic operations for physicians
• Managing physician orders and tasks in a timely manner
• Responding to patient inquiries regarding clinical care within 24 business hours
• Informing and educating patients about tests and procedures as requested by physicians
• Processing prescription renewal requests as directed by physicians in collaboration with administrative staff
• Liaising with external facilities on patient referrals
• Meeting established Key Performance Indicators (KPIs) for the role
• Utilizing Tableau reports for daily workflow
• Updating patient records and documentation with clarity and accuracy
• Collaborating with cross-functional teams and other KMH staff to assist with patient care
• Fostering a collegial and respectful clinic environment
**Minimum Qualifications:**
• Registered Practical Nurse (RPN)
• Basic Life Support (BLS) certification
**Required Skills and Experience:**
• Proven ability to work proficiently in a busy and fast-paced specialist clinic environment
• Self-directed, detail-oriented, and able to multitask with minimal supervision
• Similar experience in administration and a clinic setting is preferred
• Strong knowledge of medical terminology
• Excellent verbal and written communication skills
• Proficiency in technology, including MS Office and EMR systems experience
• Excellent time management and organizational skills
• Effective analytical and problem-solving skills
• Ability to deal with sensitive information with a high degree of diplomacy
• Team orientation
**Work Environment:**
We offer a competitive rate, comprehensive benefits, and the opportunity for growth. This is a 12-month contract role with full-time hours, scheduled for 5 days (37.5 hours) per week, on-site during the hours of 8:00 a.m.-4:00 p.m. Monday-Friday. Saturday's are required on rotation with the team for 1-2 times per month.
