Job Summary * Provide direct patient care to patients in the Cardiac Intensive Care Unit, assessing, planning, providing, and evaluating nursing care to meet the unique needs of each patient * Collaborate with patients and their families/care givers to develop and implement individualized care plans * Participate in the care and discharge planning process, ensuring effective communication and coordination with the interprofessional care team * Coordinate nursing care, prioritizing tasks and delegating responsibilities to support staff as needed * Teach patients and families/care givers about illness prevention, health restoration, and health maintenance to promote self-care and independence Responsibilities * Assess patients and collect information from various sources to inform nursing diagnoses and develop individualized care plans * Collaborate with the interprofessional care team to ensure comprehensive care planning and delivery * Participate in research and quality improvement activities to identify patient care issues and implement evidence-based practices * Act as a preceptor or mentor to new nursing staff and students, sharing knowledge and expertise to support their growth and development * Provide feedback to the Manager regarding the effectiveness of patient care programs and clinical decision support tools Qualifications * Graduation from an approved School of Nursing with current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM) * Completion of applicable post-graduate coursework or an equivalent combination of education, training, and experience * Current certification in Basic Cardiac Life Support (BCLS) Skills and Abilities * Ability to complete initial and ongoing patient assessment and provide nursing care through appropriate technical, therapeutic, and safety interventions * Ability to communicate effectively with patients, families/care givers, and members of the interprofessional care team * Ability to promote person-centered care and demonstrate empathy, sensitivity, and patient advocacy * Ability to integrate and evaluate data from multiple sources to problem-solve effectively * Ability to teach and instruct patients and families/care givers about topics essential to their health care and well-being About Fraser Health Fraser Health is a leading healthcare provider in Metro Vancouver and the Fraser Valley, serving over two million people in a diverse and vibrant community.
