Job Summary

Fraser Health is seeking a highly skilled Registered Nurse to join our team in a medicine or surgery unit. As a key member of our interprofessional care team, you will provide direct patient care by assessing, planning, providing, and evaluating nursing care to patients in acute inpatient areas.

Responsibilities
  1. Conduct comprehensive patient assessments, collect information from various sources, and make nursing diagnoses to determine the best course of action.
  2. Collaborate with patients, families, and caregivers to develop and implement individualized care plans, promoting patient autonomy and dignity.
  3. Participate in care and discharge planning, ensuring seamless transitions and effective communication with the interprofessional care team.
  4. Coordinate nursing care, prioritize tasks, and delegate responsibilities to support staff, ensuring efficient and effective care delivery.
  5. Collaborate with the interprofessional care team to identify and resolve issues, adjust care plans as needed, and address changes in patient condition.
  6. Teach patients and families about illness prevention, health restoration, and health maintenance to empower them to manage their health and well-being.
  7. Assess learning needs, provide education, and support self-care and discharge planning.
  8. Document assessments, nursing diagnoses, interventions, patient responses, outcomes, and referrals according to practice standards and Fraser Health procedures.
  9. Participate in nursing committees, care conferences, and councils as assigned or agreed upon with the Manager.
  10. Contribute to research and quality improvement activities by identifying patient care issues, collecting data, and recommending corrective actions and changes to practices, procedures, or protocols.
  11. Participate in in-services and educational programs, maintaining and updating current clinical competence and knowledge within the area of practice.
  12. Act as a preceptor or mentor to new nursing staff and students, sharing knowledge and demonstrating care activities.
  13. Provide feedback to the Manager or delegate regarding the effectiveness of patient care programs, policies, and equipment.
Qualifications

Graduation from an approved School of Nursing with current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM).

  • Ability to complete initial and ongoing patient assessments and provide nursing care through appropriate technical, therapeutic, and safety interventions.
  • Effective communication skills to interact with patients, families, caregivers, medical staff, and the interprofessional care team.
  • Ability to promote person-centered care, demonstrating empathy, sensitivity to diverse cultures and preferences, and patient advocacy.
  • Ability to integrate and evaluate data from multiple sources to problem-solve effectively.
  • Ability to teach and instruct patients and families about essential health care topics.
  • Organizational and prioritization skills to manage workload and delegate tasks effectively.
  • Ability to collaborate with and promote cooperation among members of the interprofessional care team.
  • Ability to use factual information, prior learning, and basic principles and procedures to support nursing decisions and actions with relevant research-based evidence.
  • Ability to operate related equipment, including computer applications.
  • Physical ability to perform the duties of the position.
