Job Summary
We are seeking a skilled and compassionate Nurse Care Specialist to join our team at Southbridge Care Homes. As a key member of our healthcare team, you will play a vital role in providing high-quality care to our residents.
Key Responsibilities
- Deliver resident-centered care that meets the physical, emotional, and social needs of our residents.
- Assess and monitor residents' health status, develop and implement individualized care plans, and evaluate their effectiveness.
- Supervise and direct nursing and personal care staff, providing guidance and support to ensure high-quality care.
- Collaborate with interdisciplinary teams to coordinate care and services, ensuring seamless transitions and optimal outcomes.
- Conduct performance appraisals and provide feedback to nursing staff, identifying areas for growth and development.
Requirements
- Current certificate of competence from the College of Nurses of Ontario.
- Registered/eligible for registration with the Registered Nurses Association of Ontario.
- Previous experience in long-term care, with a strong understanding of duties and responsibilities.
- Excellent communication and interpersonal skills, with the ability to work effectively with residents, families, and healthcare teams.
- Bilingualism in English and French is an asset.
