About the Opportunity

The Health Care Social Worker is a member of the interdisciplinary health care team providing collaborative, holistic care on the primary to quaternary care continuum. Social Workers are primarily concerned with the relationships between people and their environments, which are integral to psychosocial, emotional and spiritual wellbeing. The social determinants of health are embedded in social work theory and practice and are the focus for the dynamic and interactive process of engagement, assessment, planning, intervention and evaluation. Social Workers build upon the strengths of patients/clients by focusing on existing coping strategies and problem solving skills, past successes, and personal resources.

About You


•Bachelor's degree in social work from a recognized university

•Current registration with the Nova Scotia College of Social Workers

•2 years' post BSW related clinical experience required

•Previous experience working in Primary Health Care preferred

•Demonstrated ability to use a client-/family-centered approach

•Familiarity with various community-based needs of patients with chronic disease

•Demonstrated experience and familiarity with forms and resources required for certain client accommodations

•Ability to complete biopsychosocial assessments

•Excellent communication skills

•Awareness of local community resources and agencies would be considered an asset

•Valid Nova Scotia Driver's license and access to a reliable vehicle

•Ability to demonstrate effective time management and disciplined self-organization

•Demonstrated comfort and skills working collaboratively with community partners

•Competencies in other languages an asset, French preferred

Hours
  • Permanent Part-time position (.90); 67.5 hours bi-weekly
  • Shifts may include evenings, weekends and holidays.
  • This role will require the successful candidate to work from both the Mumford Professional Centre and Spryfield Community Centre, as required.
Compensation and Incentives

$ $42.68 Hourly

Successful candidates may be eligible for our benefits package which includes health, dental, travel, long-term disability, and life insurance coverage as well as a defined benefit pension plan.
