About the Role

We are seeking a skilled and compassionate Nurse Practitioner to join our team at Fraser Health Authority. As a Nurse Practitioner in this role, you will provide high-quality patient care in an outpatient setting, working collaboratively with the inter-professional care team to deliver person-centered care.

Key Responsibilities
  • Assess patients and collect information from various sources to make informed nursing diagnoses and develop effective care plans
  • Provide direct individualized nursing care to patients with varying complexities of illness, including minor surgical procedures, diagnostic endoscopy procedures, and interventional therapeutic endoscopic procedures
  • Collaborate with patients and their families/caregivers to develop and implement care plans, promoting patient autonomy and dignity
  • Participate in the care and discharge planning process, ensuring comprehensive care planning and delivery
  • Coordinate nursing care, setting priorities, assigning and delegating tasks to support staff, and making referrals to the inter-professional care team as needed
  • Document assessments, nursing diagnoses, observations, interventions, patient/family responses, outcomes/evaluations, and referrals according to practice standards and Fraser Health procedures
Qualifications
  • Graduation from an approved School of Nursing
  • Current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM)
  • Two (2) years recent relevant clinical experience in the designated clinical area and completion of post-graduate coursework and/or certification applicable to the designated clinical area, or an equivalent combination of education, training, and experience
Skills and Abilities
  • Clinical proficiency in the designated clinical area
  • Ability to initiate and maintain intravenous therapy
  • Ability to complete initial and ongoing patient assessment (clinical and diagnostic reasoning) and provide nursing care through appropriate/prescribed technical, therapeutic, safety-type interventions
  • Ability to communicate effectively with patients, families/caregivers, the public, medical staff, and members of the inter-professional care team using verbal, written, and electronic communication means
  • Ability to promote person-centered care that demonstrates empathy for patients and families/caregivers, sensitivity to diverse cultures and preferences, and patient advocacy
