Alio Health is seeking a highly skilled Bilingual Nurse Case Manager to join our dynamic team. As a key member of our operations department, you will play a critical role in managing all aspects of the reimbursement environment and program support for assigned physicians in a given geographical area.
Key Responsibilities:- Practice Management: Customize program elements to meet the needs of assigned prescribing physicians, review patient status, and assist with patient enrollment and follow-up.
- Patient Support: Welcome patients to the program, provide therapy guidelines, and ensure patient services are coordinated and tracked.
- Mentorship/Peer Support: Assist in training and development of new CM/NCM, support peers through regular communication, and develop a positive relationship with the Program Patient Support team.
- Client Interface: Develop strong working relationships with physicians and office staff, provide additional office support, and proactively resolve client issues.
- Must be a Nurse (RN, RPN, LPN) in good standing with the respective provincial regulatory body.
- English and French bilingualism.
- Case management experience preferred.
- Minimum 2 years of practical clinical experience as a healthcare professional.
