Job SummaryWe are looking for a dedicated and experienced professional to provide assessment, care planning, and client advocacy services in our Mental Health Community Clinics. The successful candidate will participate in policy and program development, health education, and promotion, and liaison with other health professionals and service sectors in the community.
Key Responsibilities- Provides assessment, care planning, and client advocacy services
- Participates in policy and program development, health education, and promotion
- Liaises with other health professionals and service sectors in the community
- Collects, distributes, and documents client information
- Facilitates client conferences and assists with active treatment phase
- Baccalaureate degree in a regulated healthcare profession
- Cardiopulmonary Resuscitation Level C Certificate (CPR)
- Non-Violent Crisis Intervention Training
- Valid Class 5 driver's license
- Registered/eligible for registration with the appropriate licensing body
- HSAS membership
- Knowledge of local human service organizations, resources, and referral procedures
