Job Summary

As a Senior Coordinator of Assessment and Care Planning, you will play a crucial role in providing assessment, care planning, client advocacy, and coordinating services and monitoring care needs. You will participate in policy and program development, health education, and promotion and liaison with other health professionals/service sectors in the community.

  • Requires a Baccalaureate degree in a Regulated Healthcare Profession

Key Responsibilities:

  • Collects, distributes and documents client information, facilitating client conferences and assisting with active treatment phase.
  • Participates in policy and program development, health education, and promotion and liaison with other health professionals/service sectors in the community.
  • Provides assessment, care planning, client advocacy, and coordinating services and monitoring care needs.

Requirements:

  • Cardiopulmonary Resuscitation Level C Certificate (CPR)
  • Non Violent Crisis Intervention Training

Knowledge and Abilities:

  • Ability to work independently and as a member of a multi-disciplinary team
  • Knowledge of how mental health impacts the individual, community and families
  • Initiative in organizing work in accordance with departmental procedures
  • Ability to support Assessor Coordinators in also following guidelines
  • Ability to adapt to changing conditions
  • Knowledge of local human service organizations, resources & referral procedures
  • Ability to clearly articulate assessment findings
  • Knowledge of the Mental Health Services Act
  • Ability to teach & train a variety of users in workshop and individual sessions
  • Demonstrated knowledge in the area of adult assessment and intervention
  • Knowledge of psychopathology and the family life cycle and systems theory.
  • Knowledge in mental health/mental illness impact on client, family and community
  • Must be well organized and able to meet deadlines
  • Ability to deliver workshops and lecture material to clients.
  • Ability to work independently and in collaboration with others as required
  • Ability to plan/organize/coordinate with other multidisciplinary team members
  • Ability to formulate referral plans and requests
  • Knowledge of how addictions impacts the individual, community and families
  • Knowledge in human behaviour, growth and development
  • Ability to be resourceful
  • Initiative in completing assigned tasks

Preferred Qualifications:

  • Valid Class 5 driver's license
  • Registered/eligible for registration with the appropriate licensing body
