Patient Care Coordinator Join our team at Burnaby Hospital as we build for the future of health care in Burnaby. As a Patient Care Coordinator, you will play a key role in providing clinical supervision, guidance and support to nursing staff, other clinical team members and support staff within the assigned unit(s). **About the Role** * Provide clinical supervision, guidance and support to nursing staff, other clinical team members and support staff within the assigned unit(s) * Responsible for the day-to-day operations of the assigned unit(s), including daily patient flow * Supports/participates in research, policy, quality improvement and risk management activities * Performs duties with a unit or hospital focus and supports program and/or regional activities **Key Responsibilities** * Applies established criteria and prioritizes admissions/intake, leads and facilitates actions related to daily patient flow, plans for patient discharge and appropriate patient placement/referral * Supervises nursing and nursing support staff and coordinates the work of the clinical team overall on a day-to-day basis * Supports quality patient care and excellence in practice by promoting interprofessional collaboration and team building * Initiates/leads/supports plan of care conferences and rounds for the purpose of optimal patient care management * Identifies learning requirements of staff and provides input into the development of learning objectives * Acts as a point person for patients and families who have concerns or questions regarding their care **Qualifications** * Current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM) * Three (3) years of recent related clinical experience in the designated clinical area, including one (1) year of supervisory experience * Completion of post graduate course work and/or certification applicable to the designated clinical area **Skills and Abilities** * Clinical proficiency/expertise in the designated clinical area * Ability to supervise and provide leadership and work direction * Ability to coordinate patient assignments, in collaboration with staff * Ability to monitor staff workload and patient factors and adjust patient assignments accordingly * Ability to establish priorities for patient care, patient flow/discharge planning and overall daily operations of the designated unit(s) **About Fraser Health** Fraser Health is a leading healthcare provider in Metro Vancouver and the Fraser Valley, serving over two million people.
