About the Opportunity

Reporting to the Health Service Manager the incumbent is responsible for working within the context of a dynamic, complex, pressure filled, client centered service culture, in accordance with established standards of professional practice, and the vision and values of the organization. The Geriatric Assessor reviews and assesses for inpatient admission to the geriatric services through extensive telephone discussions with the referring health teams and/or review of faxed applications. The incumbent evaluates and determines whether patients meet the admission criteria for inpatient admission. The assessor communicates with the physicians, managers, and clinical nurse leaders for each specific program and provides a daily report on the status of consults and provides up to date waitlist information. They are also responsible for collecting and reporting waitlist, admission, and length of stay data in collaboration with Decision Support to provide stakeholders with timely informed evidence to maximize efficient and effective use of geriatric inpatient bed resources. The incumbent also provides consultative services/presentations to teams and community health professionals, clients, and families. They establish and maintain communication/relationships with external referral sources, and they participate in evaluation and quality improvement activities /committees for programs and/or the organization as needed. The incumbent works closely with the Care Coordination Centre (C3) team to facilitate patient flow/admissions to the geriatric services in a timely manner, identifies barriers to patient flow and follows up with identified teams to ensure patient flow occurs when possible.

About You

We would love to hear from you if you have the following:

  • Bachelor's degree in a Heath Care discipline
  • Licensure with the relevant licensing body with annual registration requirement
  • Minimum of 5 years recent related experience
  • Certification in Basic Cardiac Life Support
  • WHMIS training
  • Completion of Annual Required Organization Practice courses
  • Must have comprehensive knowledge of evidence based clinical practice as it relates to geriatrics
  • Demonstrates effective communication skills
  • Exceptional ability to prioritize workload
  • Has strong problem-solving and clinical reasoning skills
  • Demonstrates initiative, is self-motivated, and works well independently
  • Competencies in other languages an asset; French preferred

Compensation and Incentives

$ $45.40 Hourly
