JOB SUMMARY:

To provide clinical practice support for registered rehab clinicians, registered nurses, registered practical nurses, and enhanced PSWs. Clinical support/leadership for the nursing/PSW team will include discipline-specific items, whereas the clinical support/leadership for the rehab team will be non-discipline-specific.

The successful candidate must be available to provide clinical support to our teams in Kitchener, Waterloo, Cambridge, Guelph, and Wellington County as needed. On occasion, support will also be required in London.

JOB RESPONSIBILITIES:

· Including Waterloo Wellington nursing, Waterloo Wellington rehab, and South West rehab: Conduct general and specialty chart audits identifying, planning, implementing, and evaluating improvement initiatives based on analysis of the results.

· Utilize results from chart audits, and other sources of evidence to identify gaps in practice (either for individuals or regionally). Develop, implement, and evaluate continuous quality improvement plans based on chart completion/quality.

· Including Waterloo Wellington nursing, Waterloo Wellington rehab, and South West rehab: Influence/monitor/maintain/improve quality of care, interpret clinical standards of practice, and provide coaching/staff development.

· Utilize results from coaching visits, and other sources of evidence to identify gaps in practice (either for individuals or regionally). Develop, implement, and evaluate continuous quality improvement plans based on clinical observations.

· Participation in the creation and presentation of quarterly safety review, as well as action items identified.

· Lead the clinical orientation and preceptorship of new staff.

· Review nursing Practice questionaire and rehab practice questionaire and help develop personalized learning plans.

· Educate, assess, develop, and maintain clinical skills and competencies for individual nurses and ensure appropriate skill mix for service delivery.

· Maintain the accuracy of employee clinical competency profiles.

· Implement annual education plan, track, and report on completion rates.

· Plan and execute annual clinical skill day events to facilitate re-evaluation of nurses' performance and continual competencies.

· Participate in knowledge transfer activities that support the introduction and utilization of clinical knowledge tools and evidence-informed practices.

· Work collaboratively with the Regional Director, South West and Waterloo- Wellington leadership team, Clinical Practice Resource Team (CPRT), and Advance Practice Leader (APLs) to improve clinical quality outcomes and define. improvement activities that result from analysis of data from formal reports.

· Actively participate in SE Health's broader Clinical Support Network supporting organizational quality and clinical excellence.

· Engage in continuous quality improvement of safe quality client care and client/caregiver experience, adopting the quintuple aim of quality framework and leading practices for measurable results and enhancing performance-driven culture.

· Works in partnership with the Regional Director/Directors, the Professional Practice Leader, and the site leadership teams to drive a culture of quality improvement and client safety.

· Contribute to the development and execution of SE Health's HOPE Clinical Model of Care at the site, ensuring the sustainability of practices for long-term success.

· To support delegation and ongoing competency monitoring of enhanced PSWs.

QUALIFICATIONS:

· A Registered Nurse (RN) designation with membership in good standing with the College of Nurses of Ontario is required.

· A Bachelor of Science in Nursing is required.

· A strong generalist clinical nursing background required.

· A minimum of one year community nursing experience required.

· A minimum of 2 years adult education experience preferred.

· Membership with RNAO preferred.

· Recent and relevant clinical knowledge and expertise of older adult and paediatric care needs.

· Ability to promote and facilitate evidence-based practice within a community setting.

· Demonstrated effective critical thinking, analytical, decision-making, and problem-resolution skills.

· Demonstrated expertise and ability to apply, teach, coach and mentor individuals and teams.

· Maintains accountability within the team to achieve work priorities within stated timelines.

· Superior communication skills and ability to communicate effectively to direct and motivate team and others, maintaining a persuasive and credible presentation style at all levels of the organization.
