About the Opportunity

Reporting to the Manager, Palliative Care Services or designate, the Palliative Care Consultation Nurse (PCCN) provides a specialized consultative nursing service to individuals who are living with a life-limiting illness. The PCCN is a member of the interdisciplinary Palliative Care Consultation Team. The PCCN completes comprehensive assessments and works in partnership with interprofessional care teams, primary care providers, community agencies and volunteers in addressing the full range of physical, psychological, social and spiritual needs of the individual. This includes a strong emphasis on the provision of skilled and timely pain and symptom assessment and management. The PCCN recognizes the patient and family as "the unit of care".

The PCCN provides clinical leadership that is focused on improving the quality of care delivered to individuals and family members coping with life-limiting illness. The PCCN serves as an educator, coach and mentor in the provision of the palliative approach to care. The PCCN exhibits sound professional judgment and initiative while working within the Mission, Vision and Strategic Direction of the organization. Palliative Care Core Competencies are incorporated into practice on a daily basis.

About You

  • We would love to hear from you if you have the following:
  • Graduate of an accredited School of Nursing Program
  • Registered or eligible for registration with the Nova Scotia College of Nursing (NSCN)
  • Minimum of three (3) years continuous palliative care consultation nursing experience preferred
  • Minimum of three (3) years continuous medical nursing experience required
  • Work experience in the care of palliative patients and families essential
  • Certification in Hospice Palliative Care Nursing (CHPCN) required or willing to become certified within two years of hire
  • Pallum LEAP Core training required or willing to complete on hire
  • BLS certification
  • Crisis Intervention Training an asset
  • Demonstrated competency in basic computer skills (Microsoft Suite, E-mail, Internet, Excel, and Meditech); and the ability to acquire additional skills as necessary (e.g. SEASCAPE)
  • Demonstrated ability to understand and utilize the domains of care associated with illness and bereavement

Permanent, Full-time / 1.0 FTE / 75 Hours Bi-weekly

Compensation and Incentives
$ $49.01 Hourly
$50.73 with 25 years of RN experience
