**Summary:** We are seeking a compassionate and experienced Registered Nurse to provide high-quality patient care in our Palliative Care Unit.

The Toronto Grace Health Centre provides specialized care and services to individuals requiring Complex Continuing Care, Post-Acute Care Rehabilitation, Palliative Care, and Transitional Care. As a Registered Nurse in our Palliative Care Unit, you will play a vital role in delivering exceptional patient-centered care, promoting health and well-being, and supporting patients and their families through challenging times.

**Responsibilities:**

* Provide excellent patient and family-centered nursing care in a complex patient environment
* Establish caring, therapeutic relationships with patients and their families
* Promote health and/or treat health conditions through supportive, preventative, therapeutic, palliative, and rehabilitative means
* Collaborate with other healthcare team members to access required services and ensure smooth transitions
* Demonstrate commitment to modeling lifelong learning and professional growth
* Possess a passion for making a difference in the lives of palliative care patients and their families and caregivers

**Qualifications:**

* Current Certificate of Competence issued by the College of Nurses' of Ontario in good standing
* Undergraduate Degree in Nursing
* Current Basic Cardiac Life Support (BCLS) Certification
* Minimum 1 year of experience within a Palliative Care environment essential
* Membership with your professional association preferred (RNAO)
* Successfully completed the LEAP or CNA Hospice Palliative Care Certification program an asset
* Strong clinical practice skills with ability to champion evidence-based best practices, particularly in pain and symptom management and end-of-life care
* Demonstrated ability to communicate effectively and problem-solve complex clinical situations and psychosocial circumstances across the continuum of end-of-life care
* Ability to recognize, assess, and treat end-of-life care emergencies and psychosocial distress for patients and caregivers
