Nurse Practitioner Opportunity

We are seeking an experienced Nurse Practitioner to join our team at Ontario Health atHome. As a valued member of our team, you will play a lead role in providing connected, accessible, patient-centred care to our patients.

About the Role

  • Assess and determine patient care needs and eligibility
  • Provide access and referrals to community services
  • Engage with patients, caregivers and other health care practitioners
  • Work collaboratively with Hospice and other healthcare professionals
  • Travel between sites in the Erie St. Clair region

Responsibilities

  • Deliver Nurse Practitioner services for the Palliative Care Consultation Team's patients
  • Diagnose, treat, evaluate, consult and manage complex pain and symptom and associated chronic disease management issues
  • Work collaboratively with Hospice and other healthcare professionals to provide recommendations and consultative guidance
  • Review and complete assessments to support challenging palliative symptom management and diagnoses

Requirements

  • Registered Nurse, Nurse Practitioner (Current registration with the College of Nurses)
  • Working knowledge of community resources and roles of health care professionals
  • Good knowledge of Ontario Health atHome priorities, policies, practices and service standards
  • Good knowledge and understanding of legislation governing health care activities
  • Knowledge of direct care/care coordination models used in community health care organizations
  • Specialized education required in palliative pain and symptom management
  • Mandatory COVID-19 vaccination policy
  • Valid Ontario Driver's License and access to a vehicle
  • Ability to operate a computer

Preferred Qualifications

  • Excellent written and oral communication skills
  • Strong interpersonal skills with the ability to provide mentorship and maintain professional relationships
  • Demonstrated interpersonal communication skills and an oral/written proficiency with Canada's two official languages
