Primary Health Care Coordinator

We are seeking a highly skilled Primary Health Care Coordinator to join our team at Nova Scotia Health Authority. As a Practice Facilitator, you will play a key role in supporting the strategic priorities of our Primary Health Care department.
The successful candidate will work closely with our leadership team, healthcare providers, and stakeholders to ensure localized support for planning, coordination, facilitation, and evaluation of Primary Health Care projects and initiatives.
You will be responsible for developing and maintaining strong working relationships with healthcare staff, provincial and other Zonal and NSH departments, external individuals, and partner agencies.
The Primary Health Care Coordinator will work collaboratively with family physicians and other healthcare providers and teams to support change management strategies that are inherent in the services plan for primary health care.
About YouWe are looking for a candidate with a strong background in health care, preferably with a baccalaureate degree from a health discipline or health-related science. You should have current registration with a professional association if applicable.
The ideal candidate will have two years of recent related experience, with a strong understanding of organizational and project management skills. You should also have experience in practice facilitation, including coaching, facilitation, change management strategies, and quality improvement.
Additionally, you should have a broad knowledge of the health system, with a high-level understanding of population health approaches, community development, primary health care, health prevention and promotion, and disease management.
We are looking for a candidate who is able to analyze information to identify priorities and emerging issues, and who has superior communication skills, both written and oral. You should also be proficient in Microsoft Word, Excel, PowerPoint, and Outlook, and have experience working in an interdisciplinary setting.
Key Responsibilities- Develop and maintain strong working relationships with healthcare staff, provincial and other Zonal and NSH departments, external individuals, and partner agencies.

- Provide practice support and advance the strategic priorities in Primary Health Care, Chronic Disease Management, and Wellness.
- Lead the planning, ongoing development, implementation, and evaluation of services and project initiatives.
- Develop and maintain a broad knowledge of the health system, with a high-level understanding of population health approaches, community development, primary health care, health prevention and promotion, and disease management.
We offer a competitive salary range of $34.11-$41.23 per hour, as well as a comprehensive benefits package, including health, dental, travel, long-term disability, and life insurance coverage, and a defined benefit pension plan.
