Key Responsibilities- Provide high-quality nursing care to individuals and families in the community, assessing their needs and developing personalized care plans.
- Teach and delegate tasks to different levels of workers and family members, ensuring seamless care delivery.
- Collaborate with healthcare teams to ensure comprehensive care and support.
- Participate in relevant committees and working groups to drive healthcare innovation and improvement.
- Registration with the College of Nurses of Ontario and a graduate from an accredited school of nursing.
- Demonstrated competencies in observation, independent assessment, and health education.
- Experience providing nursing care to adults and seniors, with a focus on community-based care.
- Strong communication and interpersonal skills, with the ability to work effectively with diverse stakeholders.
- Valid CPR C/First Aid certification and a current driver's license.
- Able to wear Personal Protective Equipment (PPE) and work in a fast-paced environment.
