As a key member of our team, you will be responsible for providing high-quality patient care, promoting health and preventing illness, and developing and implementing strategies to improve health outcomes.
Key Responsibilities:- Conduct comprehensive assessments and develop individualized care plans to promote health and prevent illness.
- Diagnose and manage the care of patients, including ordering diagnostic tests and prescribing medications.
- Develop and implement strategies to promote health and prevent illness with individuals, families, and groups.
- Deliver educational programs and workshops to promote health and wellness.
- Collaborate with healthcare teams to develop and implement quality improvement initiatives.
- Master's degree in Nursing or a related field.
- Registered Nurse (EC) certification and Nurse Practitioner (NP) Adult or Primary Care certificate.
- Current Certificate of Competence with the College of Nurses of Ontario.
- Over 10 years of experience in primary and community care and health promotion.
- Experience in nursing research and quality improvement methodology.
