Misericordia Health Centre is seeking a skilled Registered Nurse to join our team in a community-based health program dedicated to supporting seniors in their homes. The successful candidate will provide comprehensive care to participants with complex and unpredictable health needs, working in partnership with seniors, families, and healthcare agencies.
Key Responsibilities- Provide independent nursing services, including administration of prescribed medical treatments and leadership within the healthcare team.
- Facilitate coordination of participant care and conduct intake assessments and home visits.
- Collaborate with healthcare staff in the program and community to provide care to participants with complex health needs.
- Develop and implement individualized care plans, ensuring participant-centered care and promoting health and well-being.
- Communicate effectively with participants, families, and healthcare teams to ensure seamless care coordination.
- Graduate of an approved Registered Nursing educational program.
- Canadian Nurses Association (CNA) certified nurses preferred.
- Advanced preparation in gerontology an asset.
- Physical health to meet the requirements of the position.
- Ability to prioritize care for an individual participant or a group of participants.
- Ability to delegate care appropriately and monitor outcomes of delegated care.
- Ability to respond to a variety of simultaneous demands.
- Ability to maintain concentration with frequent interruptions.
- Ability to function in a demanding and stressful environment.
The successful candidate will work in a community-based setting, providing care to participants in their homes and in the program. The position requires a valid driver's license and access to a vehicle for home visits.
