About the Role

We are seeking a highly skilled and compassionate Community Outreach Support Worker to join our team at Fraser Health Authority. As a key member of our healthcare team, you will play a vital role in providing patient-centered care and support to individuals and families in our community.

Key Responsibilities
  1. Client Assessment and Support

Conduct thorough assessments of clients' needs and develop personalized care plans to ensure optimal health outcomes.

Community Outreach and Engagement

Establish and maintain relationships with community partners and stakeholders to facilitate access to resources and services.

Care Coordination and Collaboration

Work collaboratively with healthcare teams to ensure seamless care transitions and effective communication.

Client Education and Empowerment

Provide education and support to clients and their families to promote self-management and independence.

Documentation and Reporting

Maintain accurate and comprehensive records of client interactions and care plans.

Requirements
  • Grade twelve (12) plus successful completion of the Community Mental Health Worker Certificate or Social Services Worker Diploma and two (2) years' recent related experience, or an equivalent combination of education, training and experience.
  • Current CPR certificate. Current Food Safe certificate. Valid Class 5 BC Drivers Licence and access to personal vehicle with appropriate business insurance coverage.
  • Knowledge of legislation such as the Mental Health Act and the Guardianship Act.
  • Ability to communicate effectively both verbally and in writing.
  • Ability to deal with others effectively.
  • Ability to prioritize and organize client demands made and care needs.
  • Ability to problem-solve.
  • Ability to teach life skills.
  • Sound judgment and good observation skills.
  • Ability to demonstrate an appropriate level of initiative and independence.
  • Ability to work cooperatively as part of a multidisciplinary team.
  • Ability to promote positive change and independence.
  • Physical and ability to carry out the duties of the position.
  • Ability to operate related office equipment.
  • Advocacy skills.
