Job Summary

We are seeking a highly skilled Licensed Practical Nurse (LPN) to join our team at Horizon Health Network. As a key member of our multidisciplinary team, you will provide holistic care to individuals and groups of patients, working to the full extent of the LPN scope of practice.

Key Responsibilities
  • Patient Care
    • Collaborate with patients and families to develop and implement individualized care plans
    • Provide therapeutic relationships and support patients in their recovery planning
    • Act as a patient advocate, promoting open communication and professionalism
    • Maintain knowledge of treatments, equipment, and supplies pertinent to the service
    • Work collaboratively with other team members to ensure quality patient care provision
  • Educational/Human Resources
    • Maintain personal certification skills required
    • Model professionalism and maintain standards of practice and professional boundaries
    • Assist with and support the orientation of new staff
    • Assist with educating patient/families/staff/community
    • Support the preceptorship program
  • Financial
    • Maintain awareness of fiscal responsibility and promote a culture of cost effectiveness
    • Act as a change agent to enhance quality of care, work environment, and sustainability
Requirements
  • Minimum of 1 year of recent and direct experience in Addiction and Mental Health
  • Actively registered with the LPN Association of New Brunswick (ANBLPN)
  • Current BLS, Non-Violent Crisis Intervention, ASIST, and Motivational Interviewing certifications
  • High level of interpersonal skills and ability to work well within a team and independently
  • Demonstrated awareness of assessment tools and treatment modalities
  • Demonstrated knowledge of and adherence to relevant legislation and processes
